Under the changing table there is a basket with 2 diaper creams labeled “keep out of reach of children” accessible to children which is unsafe. Rm 3: There is a staff purse in the lower file drawer accessible to children which is unsafe. Rm 4: Dent in the lower part of the wall by the outside door which is unsanitary. Rm 6: There is a can of Febreze on the teacher desk labeled “keep out of reach of children” which is unsafe. Rm 6: Baseboard is missing between the closet and window. Rm 7: There are 2 staff purses on the teacher desk accessible to children which is unsafe. Rm 8: Blinds are broken and there is a hole in the closet door which is unsafe and unsanitary. There is Kroger brand soap on the sink labeled “keep out of reach of children” which is unsafe.

The outlet plastic cover, above the TV, is broken and accessible to school-age children which is unsafe. Room 6's sinks seal is broken and is not sturdy. Room's 2, 3, 4, 5, 6, and 8 have peeling painting exposing drywal which is unsanitary.
