On the back playground there are gaps between the ground and fencing creating entrappement hazard. A plank in the wood fence is bent outward creating a gap. The wood fencing is leaning in a wave pattern and not straight which is a hazard. Send documentation to consultant.
In the front 2's room, there are water stains on the ceiling and peeling paint on the walls which is unsanitary. In the front 2's room in a child's cubby there is Equate Healing Ointment lableded "Keep out of reach of children" accessible to children which is unsafe. In the back room the baseboard is coming off the wall and there is peeling paint exposing drywall through-out the room which is unsanitary.

Each child has age appropriate immunizations including Varicella and Pneumococcal vaccines. Documentation includes: -Attendance records of all children in attendance. Immunization records for each child (includes month, day and year given for each immunization and childs birth date).
