Each childcare provider, household member, employee, volunteer caregiver shall submit fingerprints for a national criminal history background check by the FBI or each childcare provider has local criminal check with documentation that national check is applied for.
Each childcare provider shall have documentation of a Drug test and result does not show presence of illegal controlled substance(s).(Standard 5 or 8 panel urine test)

At time of visit, items labeled "Keep out of reach of children" were accessible. Infant 130- Changing table with deterent (deterents are not considered locks) contained A&D ointments and Lysol Spray. Toddlers 160 - First Aid kit containing Non- Aspirin, Aspirin, Alcohol pads and eye solution was located in changing table with a deterent. Room 270 Twos changing table with deterent contained Butt Paste, and Eucerin. Room 290- Changing table with deterent contained Fabulousa.

Both restrooms upstairs had low water pressure in the girls and boys sinks. Sinks that had combo wash and dry feature had cold water only. Consultants were unable to get accurate reading due to time feature of sink combo.

The childcare facility shall have an approved source of running water from a sink that is in an area where childcare is provided. Meets IDEM standards as required.
At time of visit in infant classroom 150 6 month old was sleeping in crib wearing a headband.
At time of visit, items that were open and/or out of original packaging were not dated; Cheddar cheese, sour cream, flour, fruit in pan with reynolds wrap and omelettes.
