Corrected on 2/4/25 At time of follow up investigation, the provider completed an incident report and contacted CPS. Going forward, the staff will complete thorough checks of the restroom prior to children occupying them and will be more diligent about only letting one child in at a time.
At time of investigation, provider confirmed a child hid in the bathroom from staff and two children inappropriately touched each other.
Corrected on 10/11/24 At time of inspection, a cupboard under the sink contained chemicals and were accessible to children. Provider corrected on-site and moved chemicals up on high shelf.
