Corrected on 4/3/25 At time of follow up investigation, the responsible staff member has been terminated and the parents were notified of the missing child. Name tags have been implemented to prevent recurrence of this in the future. Supervision was in compliance at time of follow up.
At time of the investigation, provider confirmed that a child was missing and was unsupervised for a significant period of time.
