Corrected on 5/10/24 At time of follow up inspection, exemption forms have been emailed and a doctor's appointment was scheduled. Immunization records are current.
At time of inspection, provider is not current on immunization records for two children.
Corrected on 3/22/24 At time of follow up investigation, all medicines are stored out of reach and inaccessible to children. In addition, the provider will have a staff member sit in a chair outside of the napping room with the door open allowing children to be within sight and sound of staff.
