109.6(6)d: Center repeats national criminal history checks at a minimum of every four years or when aware of additional history that occurs. Fingerprints should be sent off prior to employment. CCR&R Consultant Brenda did roll prints after the HHS visit so this is in the process of correction.
109.9(1)d: All files contain a pre-employment physical exam report completed within six months prior to hire and at least every three years. Physical exams shall be documented on form 470-5152, Child Care Provider Physical Examination Report. 109.10(1)b School-age (for children five years and older and enrolled in school): Annual statement of health status signed by parent is submitted prior to admission, certifies that the child is free of communicable disease, and lists allergies, medications and acute/chronic conditions. All children do not have signed statements of health. The OSS is working with parents to secure these. In Process of Correction
109.10(15)a: The center shall have written emergency plans and diagrams for responding to fire, tornado, flood, and plans responding to intoxicated parents, and lost or abducted children. Shall have guidelines for responding or evacuating in case of blizzards, power failures, bomb threats, chemical spills, or other disasters that could create structural damage to the center or pose health hazards. If center is within 10 miles of nuclear power plant, center shall have evacuation plan. Emergency plans shall include written procedures including plans for- Evacuation to safely leave the facility- Relocation to a common, safe location after evacuation- Shelter in place to take immediate shelter when the current location is unsafe to leave due to the emergency issue- Lock down to protect children and providers from an external situation- Communication and reunification with parents or other adults Responsible for the children which includes emergency telephone numbers- Continuity of operations- To address the individual children, including those with functional or access needs. A new plan for the program should be developed including all information provided on the EPP template. The new plan has been written and shared with staff as of 11/4/24. 109.10(15)b: Emergency instructions, phone numbers, and diagrams for fire, tornado, and flood shall be visibly posted and documented at least once a month for fire and tornado. Records shall be maintained for current and previous year. Tornado and fire drills should be documented monthly. These will begin in November. 109.10(15)c: Center shall develop procedures for annual staff and volunteer training on emergency plans. Once the plan is updated staff will be trained on the plan and sign and date acknowledgement of such. Staff will sign and date acknowledgement of the new EPP in November.
109.10(16)a: The center and supervisor shall ensure that staff knows names and number of children assigned. Staff shall provide careful supervision. Staff did not know the number of children they were supervising. It is recommended children be broken into smaller groups. This has been discussed with administration at length since application. The staff all now have assigned children they are supervising. CORRECTED 10/28/24 The children they are supervising are in ratio in their 470-0724 (Rev. 02/06) Page 3 of 5 group.
109.11(3)d: Record of monthly inspections of outdoor recreation area and equipment shall be kept. This will begin in November.
109.15(2): Center shall follow minimum CACFP menu patterns for meals and snacks. Menus planned one week in advance, made available to parents, and kept on file with substitutions noted. Avoid foods with high incident rate of causing choking. No menus were available. The Director or OSS will speak to the school about providing foods that meet CACFP guidelines. They also agreed to begin keeping menus. This is important in case a child should have a food allergy. Menus were provided on 11/4/24. Some foods served do not meet CACFP guidelines. All freezers and refrigeration units should have working thermometers so temperatures can be monitored. V.
