During your renewal this past summer, you removed adult son Tyler from your registration as he had moved out of the home. He is now returning to the home and needs to be added back onto your registration as an active household member. I will check the Kindertrack system by 12/31/2024 to be sure the Registration Unit has received the request and/or has activated Tyler. No re-check is required at Melissa's operation. The above correction needs to be acted on by 12/31/2024 and I will monitor the progress through my own system check. 470-5431 (Rev. 8/17) 1305 E.
