Each dog or cat in the household shall undergo an annual health examination by a licensed veterinarian. Acceptable veterinary examinations shall be documented on Form 470-5153, Veterinary Health Certificate. This examination shall verify that the animal’s routine immunizations, particularly rabies, are current and that the animal shows no evidence of endoparasites (roundworms, hookworms, whipworms) and ectoparasites (fleas, mites, ticks, lice).
Each pet bird in the household shall be purchased from a dealer licensed by the Iowa department of agriculture and land stewardship and shall be examined by a veterinarian to verify that it is free of infectious diseases. Acceptable veterinary examinations shall be documented on Form 470-5153, Veterinary Health Certificate. Children shall not handle pet birds.
Aquariums shall be well maintained and installed in a manner that prevents children 470-5431 (Rev. 8/17) 1305 E. All animal waste shall be immediately removed from the children’s areas and properly disposed of. Children shall not perform any feeding or care of pets or cleanup of pet waste.
No animals shall be allowed in the food preparation, food storage, or serving areas during food preparation and serving times 441 IAC 110.8(4) Emergency Plans 441 IAC 110.8(4) Emergency Plans: plans in case of man-made or natural disaster shall be written and posted by the primary and secondary exits. The plans shall clearly map building evacuation routes and tornado and flood shelter areas. Findings: This worker arrived at the home on 10/18/22 around 2:15pm to find the children napping. Safe sleep procedures were observed. On this date Angela had 6 children present. She did have 4 children under the age of two and no children under the age of 12 months. She was in compliance with her numbers and age ranges for category B. A complete tour of the home was completed with the following noted:
Angela did have numbers for parents written on paper and posted and accessible near the phone but it did not include the emergency contact if parents cannot be reached and the and the child's physician.
Emergency and disaster plans for fire and tornado were not written and posted by the secondary exit. She did have one by the primary exit.
Angela did not have any non-smoking signs posted and they need to be at every entrance of the home. The outdoor area was observed with no hazards noted. A full review of records noted some items needing updates:
Angela did have annual exams for all four dogs but her dog Grace needed an updated one. Angela did report the exam is scheduled for 10/19/22. In regards to the files for the children in care, Angela's had everything she needed in the children's files. Angela's files were organized so this worker was able to look through them without the assistance of Angela so Angela was able to tend to the children in getting them up from naps and getting diapers changed accordingly. 470-5431 (Rev. 8/17) 1305 E. Angela did have numbers for parents written on paper and posted and accessible near the phone but it did not include the emergency contact if parents cannot be reached and the and the child's physician. Angela did ask why she needed to have the emergency contact and Doctor information on that written documentation as she would just call the parent. We did discuss scenarios where she might not be able to reach a parent if they cant answer the phone at work. We also discussed the need to have that information with her if she takes the children on walks or goes to the park. I shared some examples including if she were at the park and something happened to her and others needed that information. Angela agreed to update the documentation accordingly to include all the information that is required. Angela should get that updated documentation tome via a picture within the next two weeks.
Emergency and disaster plans for fire and tornado were not written and posted by the secondary exit. She did have one by the primary exit. We discussed the expectation of having the emergency and disaster plans by the primary and secondary exits. Angela shared that her husband does not wants stuff posted on the walls in the main living area of the home and we discussed a location that would be appropriate. Angela should send me a picture to verify she has that posted accordingly within the next two weeks.
Angela did not have any non-smoking signs posted and they need to be at every entrance of the home. Angela shared that she has never had these posted as no one in the home smokes. We did discuss the registration guidelines of having the non-smoking signs posted at every entrance and in a vehicle if she transports but she reported she does not transport children. Angela should send me a picture of the non-smoking signs posted accordingly within the next two weeks.
