DHHS Medical Physical Forms need to be completed every 3 years for any household member 18 years or older. I left blank forms for you to take to your doctor.
The child locks in the kitchen need to be replaced. You did text me a picture later in the day of the cleaning chemicals moved until you can replace the child locks.
Work with your CCR&R Worker to develop your policies with handling illness for example.
Emergency numbers for each child in care need to be written on paper and easily accessible. A copy of these numbers need to be in the vehicle used to transport the children.
All electrical outlets need to be capped with safety caps at all times
Each child's emergency numbers need to be written on paper and posted in an easily accessible area in the Child Development Home.
A certificate of completion for the CPR Training needs to be in the provider file. Suggestions/Recommendations: I encourage you to work with your Child Care Resource and Referral Worker (Traci Honold 515-802-8163) with the above corrections and any other compliance or training efforts. Corrective Action Required: The above correction needs to be completed by 5/13/2023 and will be checked on your next compliance visit. Non-compliance with any of the mandated requirements listed above may lead to the cancellation or revocation of your Child Development Home Registration.
