CCLW finds that from 2024 to 2025, Child #1 was bitten by Child #2 on at least four separate occasions when both children were assigned to the Toddler #2 and the Explorers Rooms. CCLW finds that Stepping Stones Academy Waipahu did not provide to Child #1's parents a written incident report on March 24, 2025 and on April 24, 2025, informing them that their child was bitten in the Explorers Room. In addition, CCLW finds that the 2024 incident report was not preserved by the facility when it implemented a new software application.<br/>

CCLW finds that at Stepping Stones Academy Infant and Toddler Center Waipahu, Child #2 was not adequately supervised "at all times" by the child's caregivers in the Toddler #2 and the Explorers Rooms. CCLW finds that once a parent or guardian places their child into the care and the custody of Stepping Stones Academy, the facility and its staff members are responsible to safeguard their child's supervision, development, safety, and protection apart from the parent or guardian under the definition of "care" in accordance with §346-151, Hawaii Revised Statutes (HRS) Definitions.<br/>

TB clearance 2 IT staff member does not have written evidence of the results of a TB test or TB assessment.<br/>
Lack of Supervision Stepping Stones Academy Infant and Toddler Center violated §17-895.1-25(a), Hawaii Administrative Rules (HAR) Program provisions.<br/> <br/> Child Care Licensing Worker (CCLW) finds that at Stepping Stones Academy Infant and Toddler Center Waipahu, Child #2 was not adequately supervised "at all times" by the child's caregivers in the Toddler #2 and the Explorers Rooms.<br/> <br/> From March of 2025 to May of 2025, one toddler was involved in nine incident reports. Child Care Licensing Worker found that the toddler staff members during that period of time did not share the child's challenging behavior with the director. As a result, those toddler staff members actions did not ensure that the child's challenging behaviors were addressed effectively, responsibly and consistently to safeguard other enrolled toddlers at the center.<br/>
