It is imperative that you complete a course immediately to avoid suspension of your certificate of registration.</b><br/>
Obtain and maintain liability insurance coverage as defined in section 346-157, HRS Please submit a copy of a current (not expired) copy of a Certificate of Liability Insurance (COI) for your facility. This requirement has been deficient since, at the least, November 2025, which is over 6 months ago.<br/> <br/> <b>Per §346-157 Hawaii Revised Statutes (HRS)(b)(c)(d)(2)Liability Insurance coverage, requirement. The department shall <u>require all providers to obtain and maintain liability insurance coverage as a condition of licensure, temporary permission, or registration to operate a child care facility</u>. <u>All providers are required</u> to <u>disclose in writing</u> to each <u>parent or guardian, within seven working days of any change, cancellation, or termination of liability insurance coverage</u>, that the coverage has been changed, canceled, or terminated while the parent's or guardian's child is care for a the provider's facility. <u>The department may suspend or revoke a license, temporary permit, or certificate of registration of a provider</u> in accordance with section 346-161 or 346-175 <u>if: (2) the provider does not comply</u> with the <u>insurance coverage</u> and <u>disclosure requirements</u> of this section and rules adopted by the department pursuant to this section.</b><br/>

Caregivers complete 16 hours of training in at least two of the department-approved topic areas within twelve months of the initial training and annually thereafter Please Update the Health & Safety Training with Parents Attentive to Children (PATCH) for you and your household members whose training periods are expired over 250 days:<br/> <br/> XXXXX<br/> XXXXX<br/> XXXXX<br/> <br/> For all of you, there is only one hour of the required annual 16 hours of training recorded.<br/> <br/> <b>NOTE: We are reminding you that all household members and caregiving staff need at least 16 hours of department approved training on an ANNUAL basis in at least two of the required topic areas. If this is not completed we can suspend your certificate of registration and you will not be able to care for more than two children unrelated to you until the Health and Safety trainings are completed.</b><br/>
Hawaii Administrative Rules All providers shall comply with the new Family Child Care Homes Hawaii Administrative Rules (HAR) effective October 1, 2024. Please note the following:<br/> <br/> -17-891.2-2 Application<br/> (2) Evidence of satisfactory physical examination and TB clearance.<br/> (4) Results of the background checks as described in section 17-801-4 Background checks (5) Employment history checks.<br/> <br/> Your substitute, XXXXX must be cleared for employment history, physical exam and TB. She shall account her employment status during July 2021 - August 2024.<br/> <br/> Note: Substitutes shall only be used for emergency situations only.<br/> <br/> Please use attached forms:DHS 984 Medical Report, DHS 959A Self-certification of Employment, and DHS 959 Employment History Clearance forms. Please contact worker, Seng Lee-Ray, should further instructions be required.<br/> <br/> <br/> -HAR 17-891.2-11 Statement of Operation Policies<br/> (12) Grievances which shall include the department information for parents to file complaints<br/> (14) Regular communications to relay information about the child<br/> (15) Suspension and expulsion<br/> <br/> <br/> -HAR 17-891.2-66 Disaster Plan for emergencies. (b) Written procedures shall address (1) through (9).<br/> <br/> Please amend and provide your policy handbook to show the above are included.<br/> <br/> <br/> -HAR 17-891.2-35 (f) Staff training: each caregiver (including substitutes) shall complete annual on-going (health and safety) training.<br/> <br/> XXXXX (16 hours) and XXXXX (16 hours) as an identified caregiver as well as XXXXX (8 hours) as your substitute shall complete the appropriate hours of training.<br/> <br/> Please submit all training certificates to PATCH so that the training information may be updated to show that this requirement is met. Please communicate to your licensing worker if more time is needed.<br/> <br/> <br/> -HAR 17-891.2-66 Disaster plan (e) monthly fire drills shall be conducted and a written drill log shall be maintained.<br/> <br/> Please provide a fire drill log showing that at least three (3) drills were conducted.<br/> <br/> <br/> -HAR 17-891.2-66 Disaster plan (h) there shall be an unexpired ABC multi-purpose type fire extinguisher in the child care area.<br/> <br/> Please provide an identifiable photo showing that you have a fire extinguisher in your home.<br/>
