Rule 591-1-1-.25(3) requires the Center and surrounding premises to be kept clean, free of debris and in good repair. Hygienic measures such as, but not limited to, screened windows and proper waste disposal procedures shall be utilized to minimize the presence of rodents, flies, roaches and other vermin at the Center.

During a Complaint Investigation & Licensing Study conducted on February 29, 2024 ; It was determined based on observation that chipped paint was observed throughout the walls and the green, orange, and yellow bookshelves located in classroom 3R Front. A ceiling tile located in classroom 5R Right side was not secured to the ceiling and drooping down. The mirror hanging above the first-left sink located in classroom 4R Front was observed to contain chipped paint at the base of the mirror. Protruding screws were further observed on the sides of the toilets located in classroom 4R Front. It was further observed that the walls on the right side of the building at the exit doors of classroom 4R Front and classroom 5R Right Side leading to playground B contained chipped paint.

Rule 591-1-1-.26(6) requires that playground equipment provide an opportunity for the children to engage in a variety of experiences and shall be age-appropriate. For example, toddlers shall not be permitted to swing in swings designed for School-age Children. The outdoor equipment shall be free of lead-based paint, sharp corners and shall be regularly maintained in such a way as to be free of rust and splinters that could pose significant safety hazard to the children. All equipment shall be arranged so as not to obstruct supervision of children.

During a Complaint Investigation & Licensing Study conducted on February 29, 2024 ; It was determined based on observation that the two white riding horses contained chipped paint located on the back-right side of playground A and the second blue swing chain was observed to be peeling on the front-left side of playground B.

Rule 591-1-1-.32(2) requires the Center maintain Staff:child ratios in mixed-age groups based on the age of the youngest group of children that includes more than twenty percent of the total number of children in the mixed-age group.

During a Complaint Closure conducted on March 29, 2022 ; It was determined based on an investigation that there were 16 one and two-year-old children in the one-year-old classroom with one staff between 7:45 and 8:30 a.m., where 2:16 was needed.

Rule 591-1-1-.26(9) requires the playground to be kept clean, free from litter and free of hazards, such as but not limited to rocks, exposed tree roots and exposed sharp edges of concrete.

During a Licensing Study conducted on June 22, 2022 ; It was determined based on observation that the following hazards were observed on the playground: -Playground A: Fallen branches were observed to the right and far left, from the entrance, near the wire fence. -Playground B: Pinecones were observed throughout the playground. Also, fallen branches and leaves were observe to the left of the entrance from the classroom. Further, the S hooks on the swings were in need of tightening on the top and the bottom. Swing one and three were observed to be rusting. At the left corner, rotting and chipping wood were observed near the blue and green xylophone. The metal cord on the xylophone was also unbraiding and accessible. There was a nail that was not flush on the edge of the platform on the left of the wooden structure just before the red and green metal slide. -Playground C: Peeling wall to the right from the exit of the building on to the playground. Also, the path from the classroom to Playground B from the 4R back classroom exit was observed to have a border leaning and in need of repair. There was a loose board with a hole in the border from the path to and from the classroom upon entrance at the gate. Exposed roots were observed from the path to the playground.
