During the time of the reinspection the Licensing Specialist observed the kitchen refrigerator is still not working only the freezer is working at this time. The provider stated she is only using the freezer at this time to store food. The provider was provided with extra time to replace the refrigerator. Per the DCF Handbook Refrigerators/freezers must have accurate food grade thermometers designed to measure cold storage temperature and these must be placed inside each refrigeration and freezer unit. Thermometers in refrigerators must show a reading of 41 degrees Fahrenheit or below, and thermometers in freezers must show a reading of 0 degrees Fahrenheit or below. The thermometer must be located in the center of the unit and be readily accessible. Thermometer temperature readings higher than specified above require further temperature testing of food samples stored in the unit using a probe type thermometer; and adjustments to the unit setting to reach and maintain the required readings must be made

During the time of the re inspection the Licensing Specialist observed that the kitchen cabinets under the sink in the kitchen have not been repaired. The cabinets have 2 missing cabinet doors and not in repair. The Licensing Specialist also observed a dark black water spot in the ceiling of the kitchen that looks like mold. The spot in the ceiling was about 2 feet around in size. Per the DCF Handbook A. 31-02 · Immunization Records FDCH/LFCCH Handbook
During the time of the inspection the Licensing Specialist observed 1 child's immunization form that is expired. Per the DCF Handbook A. The operator is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form, Parts A-1, B, and/or C, DH 680 (July 2010), or the Religious Exemption from Immunization form, DH 681 (July 2008), which are incorporated herein by reference in 65C-22.001(7)(o) and (p), F.A.C., from the custodial parent or legal guardian, within 30 days of enrollment. DH Form 680 and DH Form 681 may be obtained from the local health department. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations should provide documentation of a scheduled appointment or arrangement to receive immunizations. During the time of the inspection the Child Care Licensing Specialist observed the playground to have a metal table with one leg off and the top of the metal table was laying on the ground next to the table base. The playground had a large palm tree limb hanging down next to the fence and on the ground. The large palm tree limb has sharp needle like limbs on it. These items will need to be replaced, removed, or repaired for the safety of the children.
