    03-01 · Ratio Sufficient

    On February 13, 2024, at approximately 9:25 am after receiving a complaint about ABC Kidz Preschool the Licensing Specialist arrived at the facility. Upon arrival, after waiting a couple of minutes the Licensing Specialist was met at the front entrance by a staff member J.R and two parents. The staff member J.R who was escorting two parents out after a tour. The Licensing Specialist identified herself to the staff member J.R. and the staff member called the Owner via telephone. The Licensing Specialist explained to the staff member J.R and the Owner M.A the reason for the visit and informed them that she needed to count the children and she would then go into details about the complaint. After approximately 8-10 minutes the staff member went back into the facility and the door closed locking the Licensing Specialist out of the classroom area. The Licensing Specialist knocked on the door continuously and after about 3 minutes was let in by the staff member J.R. Once the Licensing Specialist entered the classroom area it was monitored by the Licensing Specialist that while J.R conducted the tour and was outside of the classroom at the main entrance a staff member was left alone with the 1-year-old and 2- year-old classroom alone. According to the childcare facility handbook 2.3 Ratios The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. 2.3.1 Mixed Age Groups A. In groups of mixed age ranges, where children under one year of age are included, one child care personnel must be responsible for no more than four children of any age group, at all times. B. In groups of mixed age ranges, where children one year of age but under two years of age are included, one childcare personnel must be responsible for no more than six children of any age group, at all times

According to the childcare facility handbook 2.4.1 General Supervision Requirements B. Childcare personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without childcare personnel supervision inside or outside the facility, in a vehicle, or at a field trip location

    During the time of the complaint inspection, the Licensing Specialist observed that the 17 children's cots were being stored in the bathroom approximately 1 foot away from the toilet. The staff was told to sanitize and remove the cots from the bathroom. According to the childcare facility handbook 3.6.1 Bedding and Linens Each child in care must be provided safe and sanitary bedding to be used when napping or sleeping. Nap bedding is not required for school-age children; however, the program must provide an area for children that choose to rest. G. Bedding and linens, when not in use, must be stored in a sanitary manner which prevents the spread of germs or lice from other linens.     During the time of the inspection, the Licensing Specialist observed that some food were wrapped in partially opened aluminum foil and tupperware bowls with no dates labels or identification of food. According to the childcare facility handbook 3.9.2 Food Storage Proper storage of food is essential to prevent food contamination, as well as, insect and rodent infestation. Correct handling and storage of all food is a key component in preventing food-borne illnesses. To prevent bacteria growth, cold food must be kept at or below 41 degrees Fahrenheit and hot foods at or above 135 degrees Fahrenheit. Facilities choosing to prepare food must have a designated space for food storage within the designated food preparation area or in a room not calculated as part of indoor floor space, and in an area not used for diapering.     45-02 · Background Screening Documents

    During the of the complaint inspection on 2/21/24 the Childcare Licensing Specialist Team arrived at the facility at approximately 9:30 am. At this time childcare personnel opened the front door for the team to gain entry into the facility. During this time and unscreened individual was left in the classroom with the children. The childcare personnel went back into the classroom and the unscreened individual left abruptly after being asked her information. The unscreened individual was screened on 2/23/24. According to the childcare facility handbook 7.4.1 Background Screening Documents Background screening documentation must be maintained for all childcare personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. A. Each personnel record must have a completed CF-FSP Form 5131, Background Screening and Personnel File Requirements, which is incorporated by reference in paragraph 65C-22.001(7)(b), F.A.C. B. CF Form 1649A, Child Care Attestation of Good Moral Character, which is incorporated by reference in 65C-22.001(7)(a), F.A.C., must be completed for all child care personnel at the time of initial screening or upon change inemployers. CF Form 1649A may be obtained from the Departments website at www.myflfamilies.com/childcare. C. A copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. If awaiting Out-of-State criminal history results, a copy of the DCF email informing of the individuals eligibility for a provisional hire status must be in the personnel file. D. A copy of the DCF letter/email informing of search conducted of the Floridas child abuse and neglect registry must be on record for each personnel screened between July 1st and December 15th of 2016. E. A copy of each request made to out of state child abuse and neglect registries for individuals who lived outside the state of Florida in the preceding five years. F. A copy of each search conducted for out of state sexual offender/predator registries for individuals who lived outside the state of Florida in the preceding five years. G. A copy of all background screening clearance documents for the director and owner must be included in the Departments official licensing file or in accordance with the appropriate local licensing agency requirements. H. A copy of the Juvenile Records Check with FDLE for any household members aged 12 to 17 years, if facility is located in or adjacent to home of the operator

    During the time of the complaint inspection on February 13, 2024, once the Licensing Specialist was allowed access into the classroom area the Licensing Specialist observed that two children (1) child from the one-year-old class and (1) child from the two-year-old class were not signed in on the attendance sheet. The childcare personnel then wrote 9:30 am on the attendance sheet for (1 year old) and (2- year-old). However, the children were in the facility prior to the Licensing Specialist's arrival approximately 9:25 am According to the childcare facility handbook 7.5 Daily Attendance Daily attendance of children must be taken and recorded accurately by the childcare personnel, documenting the time when each child enters and departs the program. A. Attendance devices used for the purposes of tracking attendance may be used, but personnel must ensure the accuracy of the documented attendance. Each classroom must have an attendance sheet/class roster for the group of children occupying that space. Attendance sheet/class roster must accompany the childcare personnel and the group of children throughout the day should they leave the classroom. B. The custodial parent or guardian may document the time when his/her child enters and departs the childcare facility or program. However, childcare facility personnel are responsible for ensuring that attendance records are complete and accurate. C. REMOVED. D. Attendance records must be maintained for a minimum of 12 months. E. Attendance records for Voluntary Pre-Kindergarten or School Readiness may be used, if applicable
