I did confirm that on 3/14/24 transitions were not done accordingly when the afterschool group transitioned from the bathroom to the classroom. Submit a plan ensuring staff understand when and how to do transitions. Include in the plan specific changes to how transitions will be documented
I did confirm lack of supervision in that on 3/14/24 around 3pm a child was left alone in the lobby bathroom for several minutes. Submit a plan ensuring staff will provide direct supervision at all times. Include in the plan specific items that will be put in place, such as a new way for staff to do transition and hourly counts of children in care
There were toxics stored within reach in a 3's classroom. The items were moved at the visit. Director will re-train staff on proper storage of toxics
