A video footage shows a two yrs old child crawling towards the exit to the playground door. The child care personnel tried to stop him with her foot. As he kept trying to go outside, at one point she grabbed him by the arm pits and sat him back on the floor on a harsh way

B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence

The ceiling on the 2 yr olds classroom has signs of humidity where the paint is starting to pop out. On the 1 yr olds classroom, there are some holes on one of the ceiling corners. The provider is already working to repair the roof and to correct the areas that need fix. He will let licensing know when all the work is done
Two staff from a temp agency did not have the 5-hr employment history check on file. The provider will contact licensing when it is completed on or before the due date
