The child care personnel did not record the children attendance when they entered or departed the classroom/playground
Upon the arrival of the facility the driver did not do a visual sweep of the vehicles after the children departed the vehicle. A second child care personnel was not present and did not complete the physical and visual sweep of the vehicles. This occurred for both of the vans upon arrival at the facility
