On November 24, 2025, the Office of Child Care Licensing (OCCL) received a complaint alleging improper security measures, improper staff-to-child ratios and improper staffing, not following screen time requirements, and not conducting monthly evacuation drills. An unannounced visit was attempted on December 5, 2025, to begin this investigation, and additional follow-up phone calls and written communication occurred through December 22, 2025. Based on these allegations provided by a state agency representative, this complaint is substantiated. The reporter alleged the front door was unlocked on multiple occasions. The Reporter noted they observed 11 children, including an infant, present with one staff member on November 24, 2025. Owner #1 provided attendance documentation showing eight children present, noting to LS #1 the additional children, including the infant, were left briefly by a parent who went back to their vehicle. Owner #1 noted a second staff member was taking children to their bus stop. When attempting to verify this information with the Reporter, the Reporter further stated the infant remained present for the entire duration of the Reporter s time in the facility. The reporter observed children viewing a television news broadcast rather than age-appropriate educational programming, including programming viewed by the infant. Owner #1 did not comment on this allegation when asked. The reporter also stated the most recent evacuation drill occurred in May 2025. OCCL received a copy of this evacuation drill in May 2025 as a correction due to not having completed required evacuation drills during the annual inspection on April 16, 2025. Additional concerns observed during this investigation were inaccurate staff and child attendance records. The Reporter stated on November 24, 2025, at 10:10 am there was only one staff member present, and Owner #1 stated Staff #3 left briefly to take a child to the bus stop. Staff #3 was never signed out on the attendance record. Additionally, staff attendance shows Staff #2 present all morning from 6:00 am, whereas the Reporter noted Staff #2 was not present upon arrival and did not arrive until 10:10 am. Children s attendance records for this date show eight children signed in for the entire day, but Owner #1 stated Child #2, one of the eight children recorded as attending all day, went to their elementary school that day. An additional concern observed was incomplete child files due to missing and incomplete child information cards for enrolled children. Hours of attendance are not listed accurately on the child information cards, and a total of three children were present on attendance records that do not have child information cards completed. An additional concern noted was the provider not being truthful or being inaccurate when providing information to OCCL. Conflicting information was received between the Reporter and Owner #1 regarding the number of children present and the ages of the children enrolled in the facility.
