The Office of Child Care Licensing (OCCL) received a complaint on August 18, 2023, regarding administration of medication procedures, not having a clean environment, lack of supervision, staff engaging in personal activities such as cell phone usage, unqualified staff alone with children, and not following the center policies on food restrictions. An investigation was conducted on August 22, 2023. This complaint is substantiated for not following administration of medication procedures because the center was aware that a child needed eczema cream, the child brought the eczema cream into the center, but the center could not provide a Medication Administration Record for the child. There is no evidence to substantiate the complaint for not having a clean environment because the administrator stated the center was closed for a deep clean after the children were diagnosed with a contagious disease and the center was clean during the visit. There is no evidence to substantiate the allegation of lack of supervision or staff engaging in personal activities such as cell phone usage because staff denied the allegations. There is no evidence to substantiate the allegation of unqualified staff alone with children because all the staff that are family members are qualified. There is no evidence to substantiate the allegation of the center not following their policies on food restrictions because the administrator stated the child was not served regular milk. She stated the center purchased a milk alternative after not receiving the requested milk from the parent. A concern noted during the investigation was children s daily attendance was not documented.

One staff needs annual review of abuse and neglect., orientation, a job description, two references, health appraisal with TB results, release of employment history, and service letters.
Ensure all areas are clean and in safe condition. The baseboard behind the toilet in the Pre-K bathroom needs to be repaired. The vent above the changing table in the three-year-old classroom needs to be cleaned.
Store cleaners/chemicals out of children's reach. There were chemicals stored under the sink in the three-year-old bathroom.
Repair the gate in the outdoor play area to ensure it can self-close. Move the little tikes climber into the fall zone area.
Ensure the diaper changing area is cleaned with soap and water and then disinfected after every diaper change.
