Ensure proper supervision is provided at all times. Upon arrival, this specialist observed five school-age children leave the outdoor play area, unsupervised and entered the building. The children were then seen walking to a staff member who was seated at a table inside the gym. This was discussed and corrected when staff agreed to comply in the future. Ensure proper staff-to-child ratios are maintained through the assignment of qualified staff. A staff member who is not 18 years old, and has not been fingerprinted and determined eligible was observed alone with a group of four school-age children in the gym. A second staff member who is qualified as an aide was observed taking two children to the bathroon in the gym. This was discussed and corrected when staff agreed to comply in the future.

On October 28, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding classroom capacity and the administrator not being respectful. An investigation was conducted on October 29, 2025. The complaint is substantiated for classroom capacity and the administrator not being respectful by using threatening tones and actions when speaking and interacting with others at the center. According to attendance records provided to LS #1, Group 1 s classroom has up to twelve children present at one time, but the classroom only measures for eleven children. During interviews with staff, multiple staff members reported Administrator #1 has engaged in behavior and communication that staff members have identified as inappropriate and disrespectful.
