This complaint is substantiated because proper transportation permission was not available for the school-age children who the center transports to their elementary school. Additional concerns noted were the center did not have enough staff to fulfill administrative, fiscal, clerical, cleaning, maintenance, food services, direct child care, and supervisory functions. A staff member without eligibility determination was observed alone with six one year olds. An aide was observed alone with seven children, ages two to three. Toddlers were being cared for in the same classroom as older children. A staff member who was not fingerprinted for the center was present in a classroom.

OCCL received a complaint on September 14, 2022, regarding spoiled food being served to a child and not following meal components. An investigation was conducted on September 14, 2022. There is no evidence to substantiate the complaint because staff denied allowing a child to consume spoiled food and staff stated Child #1 was served cereal, milk, and a banana even though the child showed up after the breakfast cut-off time.

OCCL received a complaint on June 23, 2022, regarding improper discipline, improper staff-to-child ratios, safe sleep violations, and failure to report accidents and injuries. An investigation was conducted on June 27, 2022. The complaint is substantiated for improper staff-to-child ratios because LS Smith observed seven children, the youngest being one, present with one staff and two staff stated they have witnessed the child care center on different occasions out of ratio. There is no evidence to substantiate the allegations of improper discipline, safe sleep violations, or failure to follow accident or injury procedures because all staff interviewed denied the allegations. Additional concerns observed were the one-year-old children were allowed to watch electronic tablets, and LS Smith observed an unauthorized adult present in the child care.

OCCL received a complaint on July 28, 2021, regarding the center not providing parents with a parent handbook and infants possibly being in confining equipment longer than permitted. An investigation was conducted on August 4, 2021.There is no evidence to substantiate the complaint because staff denied them. In addition, infants were not observed in confining equipment.
