The Office of Child Care Licensing (OCCL) received a complaint on August 10, 2023, regarding not completing accident/injury reports, positive behavior supports, and lack of supervision. An investigation began on August 14, 2023. The complaint is substantiated for not documenting accident/injuries because Owner Matos stated she did not document Child #1 s injuries. The complaint is also substantiated for lack of positive behavior supports because staff members did not intervene quickly to ensure the safety of the Child #1 and Child #2 and allowed them to physically fight for almost three minutes before intervening and Owner Matos and Staff #1 demonstrate a lack of appropriate interactions as they are observed speaking in a disrespectful manner to the two children observed fighting. There is no evidence to substantiate for lack of supervision because two staff members are observed in the video footage watching the two children physically fight. A concern noted was video footage showed child care was being provided in an unapproved space in the large family child care home.
