On September 8, 2022, OCCL received a complaint regarding incomplete child files and not following the termination policy in the handbook. An investigation was conducted on September 21, 2022. The complaint is substantiated for incomplete child files because the file was not completed prior to the child starting care. There is no evidence to substantiate not following the termination policy because Ms. Cannon denied terminating Child #1. Ms. Cannon stated Child #1 could return to care once the required items were supplied. Ms. Cannon provided text messages where she stated the child was not terminated, but could not return until the required items were supplied.
