The Office of Child Care Licensing (OCCL) received a complaint on August 28, 2025, regarding not keeping the facility clean. An investigation was conducted on September 3, 2025. This complaint is substantiated because the Specialists smelled a foul odor and observed artificial flowers, floral foam, craft pliers, and a soiled diaper on the floor in the child care room. Trash was piled up and overflowing in the child care room. There were several flies in the room. Additional concerns noted were the kitchen was dirty and had uncovered, unlabeled, and expired food in the refrigerator. Outside there was a broken gate to the playground and trash piled up and overflowing. The Specialists observed an infant asleep in a dirty pack and play, with no bottom covering. Also, a blanket and toys were in the pack and play. Additionally, the provider was not truthful when asked if there were other children in care and daily attendance had not been documented for the day.

This complaint is substantiated for lack of supervision because Child #1 stated she and her classmates are left alone when the provider takes the trash out. A concern noted was improper feeding of an infant because the licensing specialist observed an infant in his car seat being fed by a bottle propped up with a blanket
