On June 30, 2025, the Office of Child Care Licensing received a complaint regarding not following the suspension policy, improper staff-to-child ratio, unqualified staff alone with children, lack of positive behavior supports, and soiled clothing. An investigation was conducted on July 7, 2025. There is no evidence to substantiate the allegation regarding not following the suspension policy, as the facility had a behavior agreement signed by the parent. The facility handbook states that after receiving help, if a child continues to hurt others, they will be suspended for one day. There is no evidence to substantiate the allegation of improper staff-to-child ratio, as the attendance records observed for the week of June 16, 2025, were compliant and at the visit, all classrooms were following proper staff-to-child ratio. There is no evidence to substantiate the allegation of unqualified staff alone with children, as all staff were qualified or accompanied by qualified staff during the visit and based on attendance records. There is no evidence to substantiate the allegation of improper discipline because all the staff interviewed stated that they use mental health consultant techniques. There is no evidence to substantiate the complaint of a child in soiled clothing because staff stated the child was refusing to change into clothing, and they had extra clothing at the facility if needed. On December 30, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding the administrator not performing administrator duties, unqualified staff alone with children, lack of supervision, and sanitation and cleaning. An investigation was conducted on January 10, 2025. The complaint is substantiated because emails to OCCL, owner statements, and attendance records indicated the administrator was not completing administrative duties as required. There is no evidence to substantiate the other allegations based on staff interviews as the kitchen was clean and all staff during the visit and according to attendance records were qualified and eligible to be with the children. Additional non-compliances cited at the visit were no top and bottom coverings were provided for children over one-year-old, sleeping material was not nonabsorbent, and the center was using improper equipment for rest. The Office of Child Care Licensing received a complaint on June 6, 2024, for improper staff-to-child ratio, unqualified staff alone with children, lack of supervision, improper discipline, meals and snacks, diapering, outdoor area, toddler and preschool not using the outdoor area at different times, not using respectful communication, and improper sanitation and cleaning. An investigation was conducted on June 12, 2024. This complaint is substantiated for improper discipline because two children stated Staff #1 puts the children down hard in a chair and one staff member stated they have heard Staff #1 yell at the children and children have told her Staff #1 hits them. A staff member stated they were aware of a one-year-old child being in the kitchen but would not elaborate. This complaint is substantiated for improper staff-to-child ratio because upon arrival there were four staff on the outdoor play area with 33 children ages two to five. This complaint is substantiated for unqualified staff alone with children because Staff #6, an aide, stated she was left alone with children to allow staff to take other children to the restroom in the front of the building. This complaint is substantiated for toddler and preschool not using the outdoor area at different times because upon arrival the specialist observed children ages two to five were together in the outdoor play area. This complaint is substantiated for improper diaper procedures because staff stated the three-year-old classroom was changing children on the floor in the classroom. This complaint is substantiated for improper sanitation and cleaning because the mop was not hung up to dry. There is no evidence to substantiate the allegation of lack of supervision because staff denied leaving a child alone in the kitchen. There is no evidence to substantiate the allegation of meals and snack not being served because during the visit, all children received their meal and drink as required. There is no evidence to substantiate the allegation of exceeding outdoor area capacity because the measured space can have up to 52 children. There is no evidence to substantiate the allegations of not communicating respectfully because Staff #1 stated she never confronted anyone about anything, and Ms. Ulysses stated there was no altercation between staff in front of her. Several additional concerns were observed during the visit. The sleep equipment was not clean and sanitary and touching another child s equipment. A two-year-old child was using a staff s personal cell phone for screen time and the staff was on the other side of the classroom and could not be supervised. Two individuals came to the facility as visitors to see staff regarding personal issues. Trash cans did not have lids and after lunch were overflowing and not taken out in a timely manner. The two-year-old, three-year-old, and Pre-K classrooms were not following the posted schedule. Child and staff attendance was not available for the requested week.
