On August 24, 2023, the Office of Child Care Licensing received a complaint regarding lack of supervision. An investigation was conducted on August 30, 2023. The complaint is substantiated for lack of supervision because Administrator Brown stated he was aware the children went to the restroom alone after it occurred. Also, a staff member stated during opening and closing, the children are sometimes sent to the restroom alone. In addition, three children stated children are allowed to go to the restroom alone depending on their age, if they are trusted, and if they have never played around in the bathroom. Additional concerns noted were staff files were not readily accessible, and staff and child attendance records were incomplete.
