OCCL received a complaint on April 12, 2022, regarding improper staff-to-child ratio, staff not documenting their attendance, improper release of a child, not following the parent handbook, feeding the children expired food, not following the posted menu, and not following diapering procedures. An investigation began on April 20, 2022, and concluded on April 26, 2022. This complaint is substantiated for improper staff-to-child ratio, staff not documenting their attendance, improper release of a child, and not following the posted menu. Ms. McPhatter was not following proper staff-to-child ratios on 11 of the 15 days OCCL reviewed. Ms. McPhatter stated Child #2 left the facility with Staff #2, and when the child s file was reviewed, Staff #2 was not on the child s emergency card as an authorized pick up. Attendance records were incomplete because Ms. McPhatter was not signed in on any records. Also, after March 21, 2022, it is unclear which staff were present, and there were days staff were not signed out. Ms. McPhatter was not following the posted menu because she was changing the menu and was not documenting the changes. There is no evidence to substantiate the allegations of not following diaper procedures, feeding the children expired food, and not following the parent handbook because Ms. McPhatter stated diapers are checked hourly and changed as needed, no expired food was observed, and Ms. McPhatter followed her handbook by giving the Reporter five days to find alternative care.
