Licensee, household member, or staff member develops a serious health condition or is diagnosed with a mental illness that poses a direct threat to the health and safety of children. Documentation from a health care provider is required that states the person may have direct access to or work with children and any reasonable accommodations that may be required. Due to health issued disclosed by Ms. Sullivan, OCCL is requesting a new Health Appraisal be submitted.
Ensure the provider is providing the direct care for the children at all times. Ensure the emergency substitute is only used in the capacity of an emergency. The providers emergency substitute was at the home caring for children ages 9 months, 2 and 5. The substitute disclosed that Ms. Sullivan was at a doctors appointment. LS Smith explained that a scheduled doctors appointment is not considered an emergency, and the substitute may not be providing the direct care for the children. LS Smith stated if Ms. Sullivan could not return to the home by 1pm, the children's parents must be notified and picked up.
A licensee shall ensure activities, such as watching television or using a gaming device, tablet, phone, or computer, are supervised, age-appropriate, and educational. LS Smith observed children ages 9months, 2, and 5 watching Cartoon Network. This was discussed and corrected when the TV was turned off.

Ensure all household members have been finger printed and have a completed health appraisal with TB results on file. LS Smith observed a household member present in the home who was not reported to OCCL.

Ensure capacity is maintained at all times. Six children preschool age and younger, were also observed in the home at the time. The provider is a level one with a capacity of five or six depending on the ages of children. The provider agreed to comply.
Ensure the Emergency Substitute is only used for an emergency situation. LS Smith observed the emergency substitute providing the direct care for the children, while the provider was at a doctors appointment. Provider agreed to comply via phone conversation.

Ensure children's attendance is being documented daily and having a health observation upon arrival.
Ensure children are provided an alternative activity after not resting for 30 minutes. Ensure children are provided a non-absorbent mat, and have a top and bottom cover. This was discussed and the provider agreed to comply.
Ensure first aide kit is complete. Substitute was not able to produce the first aid kit for review, due to not knowing where it was located.
Ensure infants have daily logs with feeding, sleeping, and diapering documented. With 15 minute sleep checks. Child #5 needs a daily log completed daily.
Ensure capacity restrictions are followed at all times. LS Smith observed 7 children present when the capacity is 6. This was discussed with provider.
Ensure all outdoor sheds remain locked an inaccessible to children . This was corrected when the shed was locked.
Ensure renewal application is submitted 60 days prior to the license expiration.
Ensure infants are having food fed to them as required. LS Smith observed an infant bottle with cereal and baby food inside. This was discussed and corrected when the bottle was poured out, and the provider agreed to fed the infant only milk out of the bottle, and spoon feed any other foods.
Ensure child files are complete. Child #1, #3, and #4 need documented date of enrollment, and hours/days they attend. Child #5 need documented date of enrollment. Child #1, #3, #4, and #6 need updated health appraisal with immunization records. Child #3, and #4 need lead test results. Child #1, #2, #3, and #4 need parents right to know, screen time, and sleeping in another room permission. Child #2 needs mat permission. Child #5 needs an infant feeding schedule. This was corrected when all children were disenrolled.
Ensure the fire extinguisher is inspected and tagged annually.
Complete the administration of medication training and receive a certificate.
Ensure the renewal application is submitted 60 days prior to the license expiration. This was corrected when the application was received via mail on July 26, 2021.
Ensure parents of enrolled children and any tours given complete the Parents Right to Know Notice Log.
