OCCL received a complaint on July 1, 2022, regarding a lack of supervision and failure to follow accident and injury procedures. The complaint is substantiated for lack of supervision because the provider was not supervising the children during nap time to hear Child #1 tell Child #2 to stop hitting and kicking Child #3 or to see Child #2 hitting, kicking, or sitting on Child #3 s head. There is no evidence to substantiate the allegation of accident and injury procedures because Ms. Bolen denied observing any marks on the children. An additional concern noted was Ms. Bolen did not intervene quickly to ensure the children were safe when Child #2 sat on, hit, and kicked Child #3.
