On January 17, 2023, the Office of Child Care Licensing received a complaint regarding accident and injury procedures and lack of supervision. An investigation was conducted on February 20, 2023. This complaint is substantiated for improper accident and injury procedures because the incident was not documented on an accident injury form, a parent was not notified after medical attention was given for a face injury, and the parent was not provided an accident and injury report within one business day. There is no evidence to substantiate the complaint for lack of supervision because Elizabeth Howard stated she was seated on the living room floor caring for the infant approximately three feet from the room with the two children occupied in her view. During the visit, Ms. Howard maintained supervision of the children in her care.
