On August 7, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding procedures to address food allergies, administration of medication for not following the action plan of giving an EpiPen, and not notifying OCCL of a medication error for not giving the EpiPen. An investigation was conducted on August 13, 2025. The complaint is substantiated because the facility has food allergy procedures in place but failed to follow the plan by feeding the child a food they were allergic to, not obtaining an allergy action plan, not administering the EpiPen as indicated on the Medication Administration Record (MAR), and not contacting the parent or 911 immediately. The complaint is substantiated for not administering medication because the child exhibited signs of a mild reaction, becoming fussy, and a serious reaction of throwing up; the doctor s office administered the EpiPen, and the facility did not follow the MAR. The complaint is substantiated for not notifying OCCL of a medication error, specifically the failure to administer an EpiPen. Upon the child's arrival at the doctor s office, the doctor deemed it necessary to administer the EpiPen. An additional concern noted was a child had an allergic reaction, and the parent or guardian was not notified immediately after assisting the child and contacting an ambulance, as needed.

The Office of Child Care Licensing received a complaint on August 12, 2024, regarding improper discipline. An investigation was conducted on August 13, 2024. This complaint is substantiated because Administrator #1 stated she looked Child #2 in his eyes, pointed her finger at Child #2, and stated sternly, You need to get yourself together. Also, three children who were interviewed confirmed Administrator #1 and Staff #1 have yelled at children and two of those children stated Administrator #1 and Staff #1 have used their hands on children.

Ensure direct supervision is occuring at all times. LS McPhatter observed staff #1 leave the classroom with all 11 children to let 9 children out of the building to the bus. Staff #1 turned around and told the two remaining children to go back into the classroom while she stayed at the door to watch the others get on the bus. This was corrected when LS McPhatter informed Staff #1 that the children have to be with her if she intends to exit the school-age classroom threshold. On June 23, 2022, OCCL received a complaint regarding the center not being kept in a clean and safe condition, lack of sanitation, possible lack of supervision, possible mold, insect infestation, and not following health exclusions. An investigation was conducted on June 28, 2022. The complaint is substantiated for not keeping the center clean because there was one light with a substantial number of water bugs and several ceiling tiles in the side entrance and the school-age classroom needed to be replaced. The complaint is substantiated for lack of supervision because upon arrival LS McPhatter noticed that there was no staff in the preschool area where 20 children were lying down on cots, most of them asleep. LS McPhatter entered the school-age classroom and saw Staff #2 insisting to Child #1 to remove herself from the school-age room to another area that is not authorized for care. There is no evidence to substantiate the complaint of lack of sanitation because LS McPhatter observed each open classroom and the kitchen, each had a bottle prepared for soap and water and another for disinfection. There is no evidence to substantiate the complaint of insect infestation, poor air quality, or mold because there was no insect evidence of infestation, no evidence of poor air quality, or evidence visible of mold growth. There is no evidence to substantiate the complaint of not following proper health exclusions because Ms. Nazelrod explained the health exclusion policies and procedures used by the center.

OCCL received a complaint on September 20, 2021, regarding improper ratios and not following all applicable state laws and regulations by not quarantining children in accordance with the current COVID-19 guidelines. An investigation was conducted on September 24, 2021. The complaint is substantiated for improper staff-to-child ratio because according to attendance records an aide was alone with children from 6:00 A.M. to 6:30 A.M. and from 8:25 A.M. to 9:00 A.M. There is no evidence to substantiate the allegation of not quarantining children because Ms. Nazelrod denied the allegation and several classrooms were closed due to positive Covid-19 cases.
