Ensure the water temperature at all sinks is at or below 120 degrees F. The sinks located in the bathrooms in the preschool classroom read 121.7 degrees F and 120.9 degrees F. This was corrected when the water heater setting was lowered. Ensure trash cans are covered. The trash can located in the bathroom in the preschool classroom was not covered. Used paper towels were observed in the trash can.

Ensure direct supervision is provided at all times. Upon arrival, this specialist observed Staff #1 and Staff #2 in the hallway. During the walk through, Staff #1 was in the Preschool Pod back classroom with a group of napping children. When asked, Staff #1 stated she stepped out of the classroom to dispose of trash and no other staff member was in the classroom. Staff #2 was in the Toddler Pod front classroom with a group of napping children. When asked, Staff #2 stated Staff #1 was in her classroom while she stepped out to get the broom and dust pan. This was discussed with staff and corrected when they agreed to comply in the future.

Upon arrival, this specialist observed one staff member supervising two classrooms of napping children in the Preschool Pod and a second staff member was observed supervising two classrooms of napping children in the Jr. Preschool Pod. This was discussed with staff and corrected when staff agreed to comply in the future.
Ensure proper staff-to-child ratios are maintained at all times. There were nine two-year-old children in the two-year-old classroom. This was discussed and corrected when one child was moved to another classroom.
Maintain safe equipment in the outdoor play area. LS Brey observed a cracked plastic picnic table in the toddler playground. This was corrected at the visit when the table was removed from the area.
Ensure child files are complete. One child needs a completed Medication Administration Record (MAR) for EPI Pen. One child needs a completed MAR for Albuterol and Tylenol.
Ensure air fresheners are not used in any classrooms or child care spaces. LS Brey observed an air freshener in the Toddler classroom. This was corrected at the visit when the air freshener was removed.
Ensure staff files are complete. One staff person needs two documented attempts at obtaining service letters and one additional reference if no service letters are received. One staff person needs two additional references.
Ensure staff are fingerprinted within five days of their eighteenth birthday. Submit the receipt of verification from fingerprinting for one staff member.
