OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. On March 10, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding an allegation of lack of supervision. A self-report was made to OCCL on March 13, 2025. A visit was attempted on March 14, 2025 but no one answered the door to grant admission to the facility. A follow-up visit was conducted on March 19, 2025. This complaint is substantiated for lack of supervision. The camera footage received showed Staff #1 was not present in the classroom during nap time. While Staff #1 was gone, Child #3 and Child #2 were both observed being inappropriate towards Child #1. Other concerns noted were Staff #1 was unqualified and alone with children and the conflicting descriptions regarding what transpired on March 7, 2025. Based on video footage reviewed and staff statements, Staff #1 and Administrator #1 were not being truthful during this investigation. Video footage shows there were no staff in the classroom at the time of the incident. Administrator #1 initially stated on March 13, 2025, in her self-report to OCCL that she was the only staff in the classroom at the time of the incident, but when interviewed on March 19, 2025, she stated she had been in the classroom, but left the classroom to do office work, leaving Staff #1 alone with the children. Staff #1 also stated Administrator #1 was in the office leaving her alone with the children and Administrator #1 then called her into the office, leaving the children unsupervised. Additionally, LS #1 and LS #2 were unable to enter the building on March 14, 2025, and were told this was due to the children being on a walk, however, the attendance records reviewed for March 14, 2025, did not reflect the children and staff having left the building for a walk. Attendance records for March 7, 2025, also showed Administrator #1 being signed into a classroom when it was shown on video and she late r stated, she was not present in the classroom during that time. Staff #1 also left the classroom during this time which was not documented on the attendance records. Finally, the door handle at the entrance of the building was off its hinges.

On October 11, 2024, the Office of Child Care Licensing (OCCL) received a complaint for not following diapering regulations, inappropriate combining of ages, not having and implementing lesson plans as required, staff using cell phones while caring for children, and inappropriate behavior management. This complaint is substantiated for not following and implementing written lesson plans because when Staff #1 was asked what the children were doing for the day according to the lesson plan, Staff #1 did not know. The posted lesson plan did not contain age-appropriate activities. There is no evidence to substantiate this complaint for inappropriate combining of ages because toddlers were only combined with older children when 12 or fewer children in total were present. There is no evidence to substantiate this complaint for staff using cell phones while caring for children or inappropriate behavior management. While Administrator #2 stated she had witnessed staff using a cell phone in the past, all staff were required to sign a cell phone policy acknowledgement. Staff were not observed using cell phones at the time of the visit. There is no evidence to substantiate this complaint for inappropriate behavior management because staff interviewed stated for behavior management, they redirect children or talk with them on eye level and administrative staff stated they have not observed improper discipline or behavior management by staff. There is no evidence to substantiate this complaint for not following diapering procedures. Although Staff #3 placed another child s diaper on Child #1 by mistake, when Administrator #1 attempted to contact Parent #1 to discuss this mistake, Parent #1 blocked the call. Staff and administrators both stated diapers are checked and changed as required or needed. An additional concern noted at the visit is that Staff #1 was observed alone with one infant and two one-year olds without having a DEEDS qualification. This non-compliance was cited un der the annual compliance review that was also conducted on October 11, 2024.

At the time of the visit one staff member left the combined classroom to use the bathroom which left one staff member alone with seven children (ages one through four years old). This was corrected at the visit. Also, attendance records from 4/20/2022 showed that Staff #1 was alone with one child from 6:35am to 7:30am. Staff #1 does not have a DEEDS early learning certificate and may not be alone with children. Send a current staff schedule for all staff for the weeks of 4/19/2022 and 4/25/2022. Send to OCCL.

At the time of the visit, LS Flowers and Supervisor Helman observed food in the kitchen area which requires heating and clean up. Owner Avis Harrison stated that food is being heated and utensils are cleaned in the infant room sink. Send Lara Flowers the written plan for food service going forward.

On January 6, 2023, The Office of Child Care Licensing (OCCL) received a complaint regarding improper staff-to-child ratio, improper discipline, and improper care of infants. An investigation was conducted on January 9, 2023. Based on the results of this investigation this complaint is substantiated for improper staff-to-child ratio because, at the time of the visit, Staff #2 was observed alone with two children in the infant room. Staff #2 does not have a DEEDs Early Learning qualification and may not be left alone with children. This complaint is also substantiated for improper discipline because, at the time of the visit, Staff #3 was heard using a harsh tone with the children. This complaint is substantiated for improper care of infants because Staff #4 stated an infant is placed into a seat for approximately one hour when classrooms combine at the end of the day. Other concerns from the visit include Staff #2 being alone with two children without being fingerprinted for the center and determined eligible and incorrect attendance documentation because ten children were signed in on one attendance record, but there were eight children observed in the toddler classroom and two children observed in the infant classroom, and Staff #4 did not sign out when she left the building.

The youngest child present in the room was one year old. The proper staff-to-child ratio for this room is one staff to six children. This was corrected when the one-year-old child was moved to the infant classroom.
