Ratios, Group Size, and Supervision
Ensure direct supervision is maintained at all times. LS observed a 2nd-grade classroom in which a child left the room unaccompanied to use the bathroom. LS Informed staff in the room that they must accompany children to any part of the facility and maintain a staff-to-child ratio while doing so. LS observed staff at the front desk radioing to the classroom for a child, then a child left the classroom to meet their guardian at the front door, with no staff in the hallway with the child. LS observed a child leave a back classroom, run down the hallway to the front door, and give another child something that was left in the classroom. No staff were in the hallway with the child.
