Ensure children are supervised at all times. The specialist observed a staff member in room #5 in the doorway threshold watching a child use the bathroom across the hallway and the children in the classroom. A staff member was in the threshold of a door way handing an item across the hallway to a staff member in the threshold of the other doorway to receivee an item. Read A Loud Staff member was alone in the hallway with one child reading a book. Ensure read a aloud is under qualified, eligible staff supervision. This was corrected when the child and Read A Loud staff went to a classroom to read to the children.
Ensure children are under direct supervision/observation. Upon arrival LS McPhatter knocked on the door and observed staff #4 in the classroom and hallway; while there were four children in the classroom; she was covering for staff #3. Staff #1 asked staff #2 to stand in the threshold of her classroom that has two children in it and watch staff #1's classroom; that has 4 children in it; through her open door approximately 4 feet way from staff #2's classroom. Staff #2 agreed to do so. Staff #1 left her room to open the door for LS McPhatter.
