Based on interviews and documents collected the above mentioned regulation was not met, C#2 dispalyed agressive behaviour towards other children in care. There were multiple incidents, which includes a 2/22/25 incident where C#2 kicked C#1 in the face one report stated it was accdientally another report stated it was not accidentally when a child is displaying challenging/agressive behaviour in the center/classroom.

On 11/18/2024 at 10:12AM during outdoor play, C1 was walking on the obstacle course when C1 lost C1's balance and hit C1's head on a wooden log Director will sign a statment, confirming she understands the aformentioned regulation cited(fictures ,furnitures, equipment and supplies). Director will email the signed docuemnt to [email protected] It caused bleeding and abrasion the the right side of the forehead. 911 was contacted and C1's abrasion required stiches.
