Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above. While touring the outdoor activity space at 10:26 a.m., LPA's observed four screws on a wooden structure that were not fully intact. The screws were hanging outside the wood approximately 1 inch which poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 11/09/2023
