Staff-Infant Ratio, Section 101416.5, (b) There shall be a ratio of one teacher for every four infants in attendance....... Child day care centers serving infants; optional toddler program; departmental guidelines and regulations, Section, 1596.956, (4) A ratio of six children to each teacher shall be maintained for all children in attendance at the toddler program. An aide who is participating in on-the-job training may be substituted for a teacher when directly supervised by a fully qualified teacher.... LPA advised the Director to develop a plan of correction (POC) and submit proof to the Oakland Office via email or mail on or before the POC due date. The Plan shall describe how the licensee will bring their facility into compliance. It must be measurable and verifiable. During the visit, the Director stated that she is and will have a Staff Meeting individually to propose a staff plan that follows the requirements mentioned in this report. A written POC proof will be submitted on or before 11/5/2025.

, at 8:15 am, LPA observed in Room 7, S2 and S3 supervised 6 infants. In Room 8, S4 and S5 were supervising 7 toddlers. At 10:15 am, LPA observed, in Room 5, S1, S2 and S13 were supervising 11 infants. In Room 8, S4 and S12 were supervising 11 toddlers As per LPA’s Record Review at 9:15 am, S2, S3, S4, S5, S12 and S13 were not Infant Care Teachers, as per Section 101416.2(b). As a result, Room 7 was not in ratio of one teacher for every four infants in attendance, at 8:15 am. Room 5 was not in ratio of one teacher for every four infants in attendance, at 10:15 am. Room 8 was not in ratio of six children to each teacher in attendance at the toddler program. This poses a potential risk to the health and safety of children in care. TSP is FREE, voluntary, and a non-enforcement service of Community Care Licensing to assist childcare providers experiencing compliance challenges. 2. 101239 (r) Fixtures, Furniture, Equipment and Supplies: Fixtures, furniture, and equipment that have been banned or recalled by the United States Consumer Product Safety Commission shall not be used or on the facility’s premises. This requirement is not being met as evidenced by LPA observed Director remove the boppy lounger pillows out of the classroom area. Director must submit a written letter to Licensing Department stating she acknowledge the policy & procedures about banned and/or recall product in a licensed day-care facility.

Based on observation LPA observed four recalled boppy lounger pillows in the infant classroom near the cribs.
