Effective Immediately, licensee agrees to adhere to the regulations and provide care and supervision, including visual observation at all times. Licensee will watch two videos from the CCLD website regarding Personal Rights & care and Supervision, send email to LPA once completed by 12/7/24. Based on the interviews, the licensee did not comply with the section sited above, in that a child was left a car alone unsupervised which posed an immediate health, safety or personal risk to children in care. An immediate civil penalty $500 was assessed.

OPERATION OF A FAMILY CHILD CARE HOME The licensee shall be present in the home and shall ensure that children in care are supervised at all times... On 02/14/2023, one child, aged 2, was out of the sight of Licensee for approx. 25 minutes Licensee has secured the gate by adding a new velcro strap to prevent the children from easily opening the gate. Licensee stated she will sit near the gate moving forward when sitting permits. Licensee will be required to attend an office meeting without supervision when the child got out of the gate and was spotted by a witness on the corner of Forbes and Sherman Way and crossed the street on their own. An immediate civil penalty of $500 has been assessed for Absence of Supervision. An immediate risk to the health and safety of the children involved. A Type A citation has been cited.

PERSONAL RIGHTS Each child receiving services from a family child care home shall have certain rights that shall not be waived or abridged by the licensee regardless of consent or authorization from the child's authorized representative. These rights include, but are not limited to, the following: To receive safe, healthful, and comfortable accommodations, furnishings, and equipment. Licensee stated that she will review the Personal Rights of children. LPA provided licensee with a copy of the Personal Rights Regulation 102423. Licensee will be required to attend an in office meeting. On 02/14/2023, it was witnessed that Child #1 crossed the street (Forbes and Sherman Way) without supervision.
