On 1/25/24, staff observed rodent droppings in the kitchen, near the refrigerator. The Director also observed rodent droppings in the kitchen on 1/29/24.
Per Director, staff will continue to use walkies to request support in the classrooms and watch “Children’s Personal Rights in Child Care” video on https://ccld.childcarevideos.org/child-care-center-operators/ Based on documentation obtained and interviews conducted, on 01/04/23, S1 restrained C1; and on 01/17/23, S2 scratched C1 under the right arm, which poses a potential Health or Safety, or Personal Rights risk to persons in care. Each staff member will view and write a summary of their understanding of Personal Rights. Each staff should sign and date their summary and submit to LPA via email by the POC due date.
Per director, a meeting with staff will be held to review policies in place and an agenda along with attendance sheet with staff signatures will be emailed to LPA by POC due date. Based on interviews conducted, on 06/27/23, Adult 1 was not informed at pick up of C1 being bitten, which poses a potential Health or Safety, or Personal Rights risk to persons in care.

Per Director, an LIC 200A application will be submitted with the new criteria for the ages of the children to read “2 until entry into Kindergarten" and email LPA with proof of application submitted by POC date . Based on interviews conducted, the preschool was providing service to C1 who is over 4.9 years of age, which poses a potential Health or Safety, or Personal Rights risk to persons in care. Director agrees to provide written plan of how the facility will maintain infant, presschool, and school age seperate during hours of opeartion at all times. Based on observation and interviews, On 06/14/23,5 preschool, 1 school-age, and 3 infant children were comingling in the front lobby; Staff 1,2,3,4, and6 corroborated that school-age children commingle in the library with preschool children during parent pick up, which poses a potential Health or Safety, or personal rights risk to persons in care.

Based on observation, interview, and record review, on 06/15/23 at 9:02 AM LPA observed Staff 8 with 13 preschool children in classroom #3; at 9:20 AM LPA observed Staff 1 and Staff 2 with 27 preschool children in the playground, which poses a potential Health or Safety, or personal rights risk to persons in care.

Based on observation, interviews, and review of Lead water testing samples Faciilty has replaced faucets/ outlets, identified with an Action Lead Exceedance of 5.5 ppb or over. These outlets will not be accessible for use until a retest has occurred. Director will email LPA a copy of parent notification letter via email by 9/16/2022. Director will send LPA a copy of the re-test summary once completed on or before 10/14/2022.
On 9/15/2022 LPA observed Director send the 2 adults home. Adults were advised a criminal clearance will be needed before return to facility. Director provided LPA with a declaration regarding both adutls. Director will have a meeting with staff to review care and supervision Director will email LPA Lowe with Based on observations, interviews, and review of Guardian the facility did not ensure background clearance was obtained for 2 adults who were in care and supervison of children which poses an immediate health, safety or personal rights risk to persons in care. meeting agenda with staff signatures as verification by end of day on 9/16/2022.
