Director will conduct an In-Service training with staff with emphasis on Personal Rights and all facility staff will watch the CDSS video at CCLD.childcarevideos.org.on Personal Rights and discuss the topic during a staff meeting and will email LPA confirmation by 3/24/2025 Per statements obtained, staff speaks to children in a harsh, strong, firm tone, which poses a potential health, safety or personal rights risk to persons in care $250 Civil Penalty was assessed today for repeated violation within 12 months. Director signed the civil penalty form, acknowledging receipt.
The Director will conduct an in-service training with all staff on the importance of Personal Rights and ensuring compliance within this section. Copies of the training along with signatures of all attendees will be submitted to CCL by the POC due date of 12/30/2024 Per statements obtained, S1 speaks to children in a rough manner

On 10/12/2022, LPA observed Clorox bleach bottles easily accessible to day care children which were stored under the kitchen sink cabinet. This poses a potential risk to the health and safety of children in care LPA observed a child-proof knob cover on the kitchen door and child safety latches on the kitchen cabinet and bathroom cabinets.
Interviews with staff revealed that Staff #1 was hired as a Pre-K teacher however is sometimes left alone with children when Staff #2 must step out of the classroom to use the restroom. This poses a potential risk to the health and safety of children in care mailed to the State Licensing office on or before December 21, 2022. SUPERVISOR'S NAME: Rita Ramos
