The facility will submit an application for change in capacity or change the room back into the licensed space (library) for children in care. The facility will complete this by 08/17/2021. Based on observation, interview and record review, The Licensee did not notify the department of the alternations made to the facility, which poses a potential Health or Safety, or personal rights risk to persons in care.
