Based on observation, interview, record review, the licensee did not comply with the section cited above in a 12 inch kitchen knife was accessible on the top kitchen counter, which poses an immediate health, safety or personal rights risk to persons in care. Licensee will also submit a written plan to be in compliance to the San Jose Regional Office by close of business 09/26/24.
