(a)Prior to admission to a family child care home, children shall be immunized against disease as required by california code of regulation. This requirement was not evidenze by Licensee states will submit immunization card for children to LPA by 06/23/25. Based on file review 6 out of the 37 children files are missing immunization cards on file which poses a health and safety risk for children in care.

(g)(8) Each family child care home shall have a current roster of children as specified in Health and safety code section 1596.841

Licensee will provide a completed and updated roster to LPA by 06/23/25. Based on file review icensee did not have a current and completed roster at the facility which poses a health and safety risk for children and persons in care.
