Site director stated that she will walk through all the classrooms and ensure that heavy shelves, aquariums and heavy objects be anchored/braced to the wall. This will be completed no later than 3/24/2025. Director will notify the LPA via email once this is completed. Based on interview and document reviewed, a 3-year old child was taken to emergency and received stitches. A fish tank that was placed on top of play refrigerator fell on top of child. It was noted that the refrigerator was not safely braced to the wall. LPAs also observed several shelves and aquariums that were not anchored to the wall. SUPERVISOR'S NAME: Joelle Redding
