The licensee shall notify the Department in writing of his/her intent prior to making any structural changes that reduce the total amount of indoor or outdoor activity space. Such structural changes shall include, but not be limited to, room additions.
Deficient Practice Statement Based on observation, interview and record review, the licensee did not comply with the section cited above the facility as being using 2 additonal rooms that are not licensed, which poses/posed a potential health, safety or personal rights risk to persons in care.
