(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.
The facility immediately removed the basketball net from the hoop and will provide an alternative mechanism that will assist the ball coming safely through the hoop. Licensing Program Analyst (LPA) Vang observed that the nylon net on the basketball hoop had been removed. Based on interviews and record review, it has been determined that a child sustained an amputation injury to their finger while playing with a basketball hoop net due to staff failing to provide adequate visual observation and supervision. This poses an immediate risk to the health, safety, and/or personal rights of children in care. By 01/22/2026 facility will provide written proof that a protective measure has been implemented to ensure the basketball hoop is safe for the use without the net, and copy of the staff attendance sign-in sheets with the date of completion indicating that all staff members have been trained and understand children’s supervision requirements, children’s personal rights, and health and safety practices for children in care.
