By 7/18/2025 before the close of business day (5:00 PM), the Director will submit a detailed written plan to the Department on how this will be avoided going forward ([email protected]).

Based on interview and record review, LPA determined that the CCC left a child unattended in soiled clothing for over thirty minutes outside. This occured on 7/2/25 around 4:00 PM. This poses an immediate risk to the health and safety of children in care.
