Based on interviews conducted, it was determined that incidents involving Staff 1 and Child 1 were observed between 4/23/25 and 4/25/25. During this period, Staff was observed on multiple occasions force-feeding Child 1 during mealtime. Additionally, Staff 1 was witnessed displaying aggressive behaviors while attempting to redirect Child 1. This included grabbing Child 1's arm in response to the child not following instructions and exhibiting tantrum behaviors.
